An unanswered message. A tone of voice. A plan that changes. To someone else it is a minor thing, and you know that, and it does not help at all, because within seconds you are already somewhere much worse and the climb down takes hours.

What follows is usually an attempt to make the feeling stop, because at that intensity it genuinely feels unsurvivable. Cutting or burning. Drinking. Not eating, or eating until numb. Sending the message you will regret, or cutting someone off before they can leave first. Each of these works, briefly. That is precisely why they are so hard to give up, and why being told to simply stop is useless advice.

People matter enormously to you, which is exactly why the fear of being abandoned is so loud, and the effort to prevent it can produce the distance it was meant to avoid. Afterwards comes the shame, then a promise to do better, then the same sequence a fortnight later.

Many people in this position have already been through several rounds of therapy, and some have been told they are too complex, not ready, or not engaging. Linehan's starting point was the opposite: that people in this much pain are doing the best they can with the skills they have, and that the missing piece is usually skills nobody ever taught them, not willingness.

The dialectic: total acceptance and real change, at the same time

Dialectical Behaviour Therapy takes its name from holding two apparently opposite truths together. You are doing the best you can given your history and your biology, and your life will not improve unless you learn to do some things differently. Therapies that only validate can leave you stuck; therapies that only push for change can feel like one more person telling you that you are the problem. DBT deliberately does both, then teaches four sets of practical skills: mindfulness, distress tolerance, emotion regulation and interpersonal effectiveness. It is structured, it is skills-heavy, and it expects you to practise between sessions.

The surge still comes. But you have something to do with your hands for the twenty minutes it takes to peak and pass, and the morning after contains no wreckage to clear up. You can say what you need in a difficult conversation without either swallowing it or setting fire to the relationship. DBT does not make you feel things less intensely. It builds a life worth living around emotions that are unlikely to become quiet.

Why DBT works

It is one of the most rigorously tested psychological therapies for self-harm and emotional dysregulation, and it is unusually specific about what you actually do.

Strong evidence where it matters most

DBT has strong randomised controlled trial support for reducing self-harm and suicidal behaviour in people with borderline personality disorder, and it is one of the few approaches for which that can honestly be said. NICE guidance on borderline personality disorder (CG78) recommends considering DBT for women where reducing recurrent self-harm is a priority, and the 2022 self-harm guideline (NG225) recommends considering DBT adapted for adolescents with recurrent self-harm and significant emotional dysregulation.

Skills you can use in the actual moment

Much of DBT is teaching, with handouts, practice and homework. TIPP for bringing physiological arousal down fast, opposite action for an emotion that does not fit the facts, DEAR MAN for asking for something clearly. Concrete tools matter when the problem is that your capacity to think has temporarily left the building.

Validation is built into the method, not added as manner

Linehan's model holds that many people with severe emotional dysregulation grew up in environments where their internal experience was routinely dismissed, so validation is a technique the therapist is trained in and supervised on, not just a personal warmth. For people who have been called attention-seeking or manipulative by previous services, this is often the part that makes the rest usable.

Knowing what full-programme DBT is

Standard, or comprehensive, DBT has four components running together: weekly individual therapy, a weekly skills training group, between-session phone coaching for crises, and a consultation team that supports the therapists. It typically runs for around a year. In the UK it is mostly delivered by NHS specialist services and a small number of private DBT teams, because no single therapist can provide a group and a consultation team alone.

Knowing what DBT-informed therapy is

Most private therapists offering DBT are offering individual therapy that uses DBT skills and principles, without the group, the phone coaching or the team. That can be genuinely useful, and it is not the treatment the headline trials tested. Ask any therapist directly which one they provide. A straight answer is a good sign; the honest ones volunteer it.

Commitment, targets and the diary card

Early sessions agree what the therapy is for and in what order. DBT works to an explicit hierarchy: life-threatening behaviour first, then anything that undermines the therapy itself, then what damages quality of life. You track urges, behaviours and skill use on a diary card between sessions, which becomes the agenda for the next one.

Learning the four skills modules

Mindfulness, the core skill of noticing what is happening without immediately reacting. Distress tolerance, for surviving a crisis without making it worse. Emotion regulation, for reducing vulnerability and changing emotions that do not fit the facts. Interpersonal effectiveness, for asking, refusing and keeping self-respect. In a full programme these are taught in a group over roughly six months and usually repeated once.
