- Takes about 3 minutes to complete
- 100% free and anonymous — no email or account required
- Built-in safety alert with UK crisis-line signposting
- Used widely in NHS CAMHS and global research
- Adolescent depression is treatable — most teens improve with the right support
About the online PHQ-A
The PHQ-A is the same scientifically validated PHQ-9 used in NHS GP practices around the country, but with wording adapted for teens. It captures the core symptoms of depression — mood, sleep, appetite, energy, concentration, self-worth, and self-harm — in just 9 items.
This free version uses the standard 9 scored items. You answer how often each problem has bothered you over the last 2 weeks. You then get a total score from 0 to 27 plus a severity band, with UK-specific signposting for support.
